Q1: What’s your favorite cosmetic procedure to perform?

Dr. Miami: My favorite is rhinoplasty, I’ve said that before, because I find it very challenging it’s like high stakes surgery it’s in the middle of your face, there’s a lot of detail, I’m very focused and no two noses are the same it’s one of those operations it takes your entire career to really master. The easiest is breast augmentation. 

Q2: What’s the best surgery for hip dip removal?

Dr. Miami: The best is fat transfer because there are no implants you can put in the hip dip, and moving fat from your love handles into the hip dip works really well.

Q3: Can you redo a tummy tuck?

Dr. Miami: Yes, you can redo a tummy tuck. I’ve had patients who have had multiple tummy tucks over the years, but there must be enough skin to pull down to do the second tummy tuck. If it’s too tight, then it might be an issue. But, for example, I’ve had patients who have had babies, had tummy tucks, they think they’re done having babies, and then have another tummy tuck. So it is possible to have another tummy tuck.

Q4: Is it dangerous to do a tummy tuck, lipo 360, and a fat transfer at the same time?

Dr. Miami: That’s a question I get a lot. Listen, surgery is like driving a car- it’s dangerous in the wrong hands. If you have a drunk driver, a car can be very dangerous. But in the right hands, that operation should be as safe as any other plastic surgery operation. I’ve done thousands of those procedures and combinations- no one ever died. I can't speak for all the practices around the world- nobody can. But in the right hands, it should be as safe as anything else.

Q5: What about having high blood pressure? Could this be dangerous when getting a cosmetic procedure?

Dr. Miami: Yes, you can’t have surgery if you have high blood pressure. Your blood pressure must be well controlled before you think about having any elective surgery. So that means that if you’ve been ignoring it, you have to get on medications, or change your diet, or exercise etc, I’ve had patients come in for surgery, and anesthesia will take your blood pressure, and if you can’t get it down and stable before surgery, they’ll cancel it. So it’s very important because it could be dangerous. The danger is that if your blood pressure is high during surgery, you can’t get it down, you can have a stroke. 

Q6: Do you have to wait until you stop breastfeeding to have a breast lift?  

Dr. Miami: Yes, you cannot be actively breastfeeding and have a breast lift. The reason is that when the milk is there, when you cut through those glands and ducts and do the lift, some of those milk ducts can get trapped, and then you’ll have like a lactoseal, like a balloon, like a blister full of milk. So it’s better to wait until your breasts dry up before you have any breast surgery, but especially a breast lift.

Q7: What pre-op test results would bump someone’s surgery back?

Dr. Miami: For example extreme anemia, high white count, low platelets, hemoglobin a1c is a big one, some people don’t even realize it til they do their pre op but they may be pre diabetic or diabetic, liver function tests can be abnormal, thyroid like I said before can be abnormal, all those things need to be rock solid before surgery. 

Q8: What is the downtime for a breast augmentation?

Dr. Miami: Most patients, for example, who fly into Miami for a breast augmentation, will fly out 36 hours later. Most people don’t drive for a few days after that. After that, you go back to your normal life. You just can’t lift anything heavy, you can’t work out, and you can’t go back to your normal sex life for a month; other than that, you’re pretty much back.

Q9: How long on average is recovery time for a mommy makeover?

Dr. Miami: After a mommy makeover, most patients take 2 weeks off from their normal work and mommy life, but it’s really about a month before you can really show it off and get back to normal 100% By two weeks, most people go back to work and driving.

Q10: What’s the ideal biggest BMI?

Dr. Miami: Liposuction is not an operation for weight loss; it’s for body contouring. So you really want your BMI below 32-33 at the most for liposuction. And your skin has to be tight because if you liposuction an area of fat and the skin is loose, it’s just gonna hang or look wavy. So it’s better to lose the weight first as much as you can and then use liposuction for contouring and body shaping.

Q11: When you get injected during a surgery, does the spot that you have a hole smell really bad?

Dr. Miami: No, I don’t know where that myth started. Fat transfers, BBLs, they do not smell. You need to just clean yourself normally, and you will not smell.

Q12: What’s the most popular cosmetic surgery you perform?

Dr. Miami: The most popular procedures are the mommy makeovers and the skin removals. A lot of people have lost weight on the GLP1s in the last couple of years, and so they’ve got a lot of skin. So that’s the most popular operation. 

Q13: What do you recommend for scar care?

Dr. Miami: I like silicone tape. For the first months after surgery, the scars are just covered with surgical tape. Once the scars come off, I would like you to use silicone tape (which is something you can get on Amazon) and biocorneum cream. I like you to alternate: use the silicone tape at night for the entire first year and the biocorneum during the day. 

Q14: Does everybody get fibrosis after lipo?

Dr. Miami: No, not everybody does. A lot of people get some degree of fibrosis by the nature of the surgery itself. When you cut yourself on the outside, you have a scar, and when you do liposuction on the inside, there’s a scar, so to speak, which is fibrosis. But just like some people scar really well, you can barely see the scar. It’s like a little line. Some people don’t scar really well on the inside either, and that’s when you get fibrosis that’s more noticeable.

Q15: Do thin women with larger implants need an internal bra?

Dr. Miami: The answer is it depends. Probably half the patients with larger implants and thin skin would need an internal bra, and it’s not so much to avoid the double bubble; it’s just to avoid the implant sliding down.
